Grant Application

  The application process is a one-page form designed to be as simple as possible. Please complete to the best of your ability and include a personal photo and any other information you feel is important to your request. We prefer a referral from a physician or physical/occupational therapist. This grant program is designed to be the funding of last resort. Please exhaust all other funding options before applying. We will work with other funding agencies and offer partial grants to purchase equipment. Direct funds will not be provided to the individual. We are a small organization and therefore, the maximum amount for each application is $5000.00. Please provide cost and available options of where to purchase the equipment. Applicants will be notified within two weeks after the Board meets with a goal of funding within 30 days. 

Applications are accepted year-round and the Board of Directors meet quarterly to review and make determinations regarding which applicants the Foundation will best be able to assist.
